5PM was the first full software product idea that I had that we were able to develop.
5PM was a post-acute care marketplace that connected patients and their families to post-acute care providers, streamlining the discharge process and putting care back into the hands of the patients.
5PM was created to put patient care back into the hands of the patients and their loved ones by removing barriers that often occur during the discharge process.
Patients were able to use the platform to make informed decisions that best suited their care needs. We were able to partner with some of the top hospitals, skilled nursing facilities, healthcare facilities, and providers around the SF Bay Area.
